Does a Crown Vic front end fit 68 D100?

I just got me a 68 D100 long bed pickup. I would like a more modern drive train and leave the body rustic. I’ve read about the crown Vic front end swap but want to make sure that works. I’d also like to add a more modern 5 speed manual behind the 318, any recommendations? And is there a rear end with disk brakes that would bolt in or go in with minimum modifications?
