If you require a PTO that offers both forward and reverse, then your part numbers would be as follows:
RL6D-U6813-L1BX PTO w/ 1.25 inch diameter output shaft, eye bolt for lever shift, shaft to the front of the truck, PTO output percentage will be 33% of engine speed and 27% of engine speed

If your winch has provisions for direction change and you only need input in one direction, then your part numbers would be as follows:
TG6S-U6818-C1BX PTO w/ 1.25 inch diameter output shaft, cable shift including cable, shaft to the front of the truck, PTO output percentage will be 40% of engine speed.

Please check the installation manual carefully, when installing a PTO with adapter, the proper backlash has to be set between the PTO and the adapter as well as between the adapter and the transmission.
